Case Management

Vector 1
The emotional and psychological challenges people face — including mental health conditions, substance use, and major life transitions — often affect daily functioning at home, work, and in relationships. While therapy supports insight and healing, translating that progress into real-world change sometimes requires additional, practical support.

How Case Management Works

Maple Cove Therapy offers case management services to help clients apply the work done in therapy to their everyday lives. Case management focuses on strengthening structure, coordination, and follow-through across personal, professional, and social environments.

Our team works collaboratively with clients and their existing clinical providers to support functional progress outside the therapy office. Case management services are non-clinical in nature and are designed to complement — not replace — ongoing psychotherapy, psychiatry, or other treatment.

Services may include care coordination, goal setting, skill development, support with organization and routines, communication with providers and family (when appropriate), and assistance navigating complex systems. Support is individualized and adjusted over time based on each client’s needs, strengths, and stage of change.

Case management works best when clients are actively engaged and committed to the process. With steady support and accountability, many clients experience increased independence, improved daily functioning, and a stronger sense of stability and confidence in their lives.
